INSTRUCTIONS

  • You are currently being assessed for attention deficit disorder with or without hyperactivity (ADHD).
  • During this process, your doctor needs the following questionnaires to be completed by the people who know you best (your parents and a teacher). You will be asked to complete other questionnaires on your own.
  • Your participation is very important.
  • There are no right or wrong answers. You will be asked questions about how you function in different situations. If you are not sure of the answer, go with the first one that comes to mind. Individual items are less important than the overall portrait.
  • If you live in two homes, it is important for these questionnaires to be completed by the people living in each home./li>

For the following questionnaires: Print 2 copies and have each parent complete the questionnaire.

For the following questionnaires: Print 3 copies and have the questionnaire completed by each parent and a teacher who knows the child well.

For the following questionnaire: Print 1 copy and have it completed by a teacher who knows the child well.

For the following questionnaires: Print 1 copy and have it completed by the adolescent if he or she is willing to do so. These tools are generally used for adults.

For the ADHD medication form: Print 1 copy and complete it if you are currently taking medication for ADHD. You can ask your parents for help. If you are not taking medication, you may use this form later if necessary.
