  • You are currently being assessed for attention deficit disorder with or without hyperactivity (ADHD).
  • During this process, your doctor will ask for the following questionnaires to be completed by people who know you best (spouse, family member, friend, roommate) and by you yourself.
  • Your participation is very important.
  • There are no right or wrong answers. You will be asked questions about how you function in different situations. If you are not sure about the answer, go with the first one that comes to mind. Individual items are less important than the overall presentation.

For the following questionnaires: Print 2 copies, complete it as best as you can, and also have it completed by someone who knows you well.

For the following questionnaires: To be completed retrospectively in order to assess how you were when you were a child. Print 2 copies, complete one yourself and wherever possible have the other completed by someone who knew you as a child and who can describe how you were at that time.

For the ADHD medication form: Print 1 copy and complete it if you are currently taking medication for ADHD. If not, you may use this form later if necessary.
